Team Leader - Practice Development Nurse: Care Homes Support Team

This is an ideal opportunity to develop your skills and provide effective professional leadership to the Care Homes Support Team, and monitor standards of education, care and professional practice. You will be helping to improve standards of palliative and end of life care for all residents, irrespective of diagnosis, within care homes in Scarborough, Whitby and Ryedale.

Saint Catherine’s cares for adults with terminal illnesses and their families and carers, covering a large geographical area of approximately 1600 square miles encompassing the Boroughs of Scarborough, Ryedale, Whitby and parts of East Yorkshire.

Saint Catherine’s is a well-loved organisation embedded in the community it serves with around 175 members of staff and over 500 volunteers. Our annual budget is in the region of £5.5m and we receive about 35% of our funding from statutory sources. Our eleven shops and fundraising teams help us raise the remaining funds from our loyal and supportive communities.

Hours: 37.5 hours per week (predominately 9am-5pm Monday-Friday with some flexibility for service delivery). 30 hours will be considered.

Job Purpose

  • To provide effective professional leadership to the Care Homes Support Team, and monitor standards of education, care and professional practice.
  • To improve standards of palliative and end of life care for all residents, irrespective of diagnosis, within care homes in Scarborough, Whitby and Ryedale.
  • To develop working relationships between care home staff and other relevant health and social care professionals

 

Essential Qualifications, Skills, Knowledge and Experience

  • Registered Nurse (Adult/General) with a valid current registration
  • Leadership qualification (or willing to work towards) and /or experience
  • Palliative care qualification (SCH accredited course 931, or equivalent qualification at diploma level or above)      
  • Teaching and assessing qualification 998 or equivalent experience
  • Knowledge and experience of clinical and information governance
  • Knowledge and experience of safeguarding procedures
  • Ability to delegate appropriately and develop other team members
  • Excellent communication skills
  • Excellent presentation skills
  • Ability to prioritise, work to deadlines and excellent organisational skills
  • Significant post registration experience
  • Significant clinical experience in Palliative Care
  • Experience of teaching and assessing competence
  • Experience of implementing and sustaining service improvement
  • Experience of Multidisciplinary team working and working collaboratively across health, social and voluntary care organisations   
  • Experience of delivering education (informal and formal)
  • Involvement with audit and measuring outcomes, project management or change management
  • Driver with full current licence.
